Do you understand that the program is voluntary and that you are under no legal obligation to participate in the Insurance Fraud Intervention Program?

Do you understand that you are suspected of committing a criminal offense in violation of your State’s statute?

Do you understand that if you are charged in Criminal Court for this offense, you have the following rights:

• the right to a speedy trial and to contest in court any accusations made against you,

• to confront and cross-examine your accusers,

• to summon witnesses on your behalf,

• the right to remain silent and to be represented by a lawyer.

Do you agree that the facts of your case might result in a finding of guilt if tried in court?

The terms and conditions of the Program have been fully explained to me. I hereby state that I am eligible for the Program and I want to apply for it at this time. No threats or promises have been made to me. I am not under the influence of alcohol or drugs at the time of this agreement.
