Non-Recidivism Agreement

 I affirms that through my participation in the Program, I have sufficiently learned and understood:

1.  What constitutes **insurance fraud** under applicable laws and regulations.
2.  The **personal consequences** of committing insurance fraud, including potential criminal liability, financial penalties, and reputational harm.
3.  The **social consequences** of insurance fraud, including its impact on families, businesses, and the broader community.
4.  The **community-wide impact** of insurance fraud, including increased costs, diminished trust, and harm to public resources.

I acknowledge that I voluntarily entered into the Program in lieu of criminal investigation. I further understand that my participation was intended to provide education, intervention, and an opportunity for rehabilitation.

I understand and agree that: If I commit insurance fraud in the future, my participation in the Program will be admissible as **evidence** in any subsequent investigation, prosecution, or administrative proceeding.

My completion of the Program demonstrates that I was specifically educated about insurance fraud and its consequences, and therefore any future violation will be treated as a **knowing and willful act**.

I attest that I will refrain from committing insurance fraud in the future. I acknowledge that this attestation is a binding commitment to lawful conduct and ethical responsibility.

I hereby declare that I have read, understood, and voluntarily agreed to the terms of this Non-Recidivism Agreement. I affirm that I will not engage in insurance fraud in the future and that I fully understand the consequences of doing so.
